Dementia Home Care

Private home care for Alzheimer's/dementia must be customized on a case-by-case basis. Each individual has unique health issues, families, experiences and memories that shaped who they are today. Our dementia care assessment process begins with the individual and their current reality. Home care plans must address an individual’s exposure to risks and strategies for preserving their safety.

Dementia Home Care Plan

Dementia home care must meet the expectations and goals of the family. By creating a home care plan, we share our observations and identify the areas that will assist the client(s). The home care plan for someone with Alzheimer’s or related Dementia must evolve as one’s health changes, which is why reporting occurs weekly and in some cases, daily. Training for the caregiver is a never ending process.

As changes occur, there must be adjustments and strategies to manage difficult behavior, for example:

  • The cost of home care requires an assessment.

  • Clients with violent behavior challenges will cost more than those who do not exhibit the same challenges.

  • We have found that the key to finding the right Personal Support Worker beyond their experience is a personality assessment.

  • Matching client’s personality with a caregiver(s) is critical if we want to maintain consistency and trust.
