Home care preserves the independence and dignity of the client and their family.  This is accomplished by creating a personalized care plan and a functional assessment addresses the specific needs, expectations, and the goal of the client and their health care team.

 

Personal support workers, registered practical nurses, nurses and client care managers are organized on a case-by-case basis to deliver the optimum level of assistance.  This is reassessed as frequently as every week based on needs and expectations of the client.
